Our experience has been serving over 100 clients in 19 states. From large continuing care retirement communities in the city to small rural long term care providers, we deliver value to improve your program. We currently are providing development, management or marketing services to these locations:
- Apostolic Christian Home is a continuum of care campus offering independent living, assisted living, and skilled nursing services in Sabetha.
- Cheyenne County Village located in St. Francis a 30-bed skilled community along with newly built ten assisted living residences.
- Dawson Place, located in Hill City, is a 36 bed community that offers long term care. Our partnership with Dawson Place was new in January 2025.
- Frankfort Community Care Home is a 40 bed skilled nursing facility with a great team and a supportive community.
- Lincoln Manor is a 45 bed skilled nursing community that truly is a small family. Grace Team partners with Lincoln County to ensure community members don't have to leave to receive care when they need it.
- Logan County Senior Living located in Oakley is a 30 bed community that offers skilled nursing as well as long term care.
- Park Lane Senior Living of Scott City is a beautiful campus offering a continuum of care. With 65 beds dedicated to skilled nursing, 11 for residential healthcare needs, and 20 independent living apartments, seniors are able to receive various services as their needs change.
- Prairie Homestead Senior Living is a homelike setting on 26 acres with all the conveniences of the city of Wichita just blocks away. Prairie Homestead Senior Living offers 56 twin homes, 24 newly constructed patio homes, plus 30 assisted living apartments.
- Sterling Village offers 45 skilled nursing beds and 12 senior living apartments in Sterling.
- SunPorch of Dodge City offers 45 beds for skilled nursing and six apartments in Dodge City.
- SunPorch of Smith Center is a new campus consisting of 2 Greenhouses of 11 skilled beds each, and 12 assisted living units. Interested to see an innovative approach to care? Sunporch of Smith would be the place to tour!
- Topside Manor of Sherman County is a 45 bed skilled nursing community that recently replaced their outdated, institutional building with 3 small homes (15 beds each), all connected with a central, common space would be home to elders for years to come. Grace Team provided development services during the building of their new campus as well as continued management services for Topside Manor.
- Winfield Rest Haven is a 41 bed skilled nursing community in Winfield. Five independent living apartments are also available for those needing less care
